As a skilled healthcare professional, we are seeking an experienced Triage Nurse to join our team at Community Healthcare of Texas. About the Role

This role requires a proactive approach to managing crisis situations, prioritizing calls, and assigning tasks to on-call staff. You will be working closely with patients, families, and colleagues to provide exceptional support and service.

Responsibilities

  • Respond to all after-hours and/or weekend calls from sites, prioritize calls, and assign crisis visits to on-call staff.
  • Distinguish needs, advise, and prioritize the need to assign on-call staff.
  • Maintain a calm demeanor under pressure while providing excellent customer service.
  • Follow up on requests and ensure timely resolution.
  • Prepare complete reports on all activities for each site following each triage day.

Requirements

  • RN licensure in the state of Texas.
  • Minimum 1 year of nursing experience required.
  • Hospice experience strongly preferred.
  • Proficient skills in computer use are essential.
