As a skilled Triage Nurse, you will play a critical role in delivering high-quality patient care and resolving crisis situations at Community Healthcare of Texas. This role requires strong leadership skills, effective communication, and the ability to work collaboratively with colleagues.
About the Role
You will be responsible for responding to all after-hours and/or weekend calls from sites, prioritizing calls, and assigning crisis visits to on-call staff. Additionally, you will provide clear and concise advice to patients, families, and colleagues, assess needs, prioritize tasks, and delegate responsibilities effectively.
Key Responsibilities

- Provide expert guidance and support to patients, families, and colleagues.
- Develop and implement effective plans to resolve crisis situations.
- Maintain accurate records of all activities, including calls, responses, and follow-ups.
- Collaborate with colleagues to ensure seamless delivery of services.
Requirements and Qualifications
- RN licensure in the state of Texas.
- Minimum 1 year of nursing experience required.
- Hospice experience strongly preferred.
- Excellent leadership and communication skills.
Salary and Benefits
