Work Schedule: Sunday – Wednesday, 1:30 pm - 12:00 am, or Wednesday - Saturday, 1:30 pm - 12:00 am. Paid on-call rotation for overnight shifts is required.

Position Summary

A Case Manager is essential for our Mobile Crisis Intervention Team (MCIT). The Case Manager engages in outreach and support activities for individuals experiencing behavioral health crises or those hesitant to seek treatment. This role focuses on building trust and rapport with clients and their families, demonstrating empathy and care to facilitate access to necessary treatment. The Case Manager operates primarily in the community, collaborating with crisis personnel and treatment staff, as well as community partners. Responsibilities include screening clients, addressing immediate concerns, connecting individuals and families to essential services, and assisting with treatment and support needs.

Key Responsibilities

  • Collaborate with crisis and outreach staff to respond to individuals or families in need, particularly those facing behavioral health crises.
  • Work alongside team members and community partners to deliver services and support.
  • Conduct screenings and outreach activities for community members requiring behavioral health services or resources.
  • Engage clients and connect them to appropriate services and support systems.
  • Travel between various sites in the community and provide client transportation as needed.
  • Utilize de-escalation techniques to support individuals involved in behavioral health crises.
  • Employ strong communication and interpersonal skills to ensure a positive experience for clients accessing services.
  • Implement motivational interviewing techniques to encourage clients to engage in necessary services.
  • Assist clients in overcoming barriers to accessing services, such as transportation issues or appointment reminders.
  • Utilize agency-designated suicide screening tools in the field and consult with a Qualified Mental Health Professional as needed.
  • Educate clients about available services and gather relevant information.
  • Coordinate care activities to ensure clients in crisis have follow-up appointments with the MCIT.
  • Provide skills training and family coaching when appropriate.
  • Offer backup coverage during workforce shortages.

Qualifications

A Bachelor's degree in a human services field or three years of relevant experience working with individuals with emotional, mental, or developmental disabilities, or a combination of education and experience that equips the candidate with the necessary skills and knowledge for the role.

Must be a Qualified Mental Health Associate (QMHA) registered with the Mental Health & Addictions Certification Board of Oregon (MHACBO) or meet eligibility requirements for registration.

Proficiency in using email, composing documents, and navigating Electronic Health Record (EHR) systems is required. Familiarity with Microsoft Word and other Office applications is preferred.

Must pass state-required background and DMV checks, possess a valid driver's license, and maintain a satisfactory driving record for client transportation. The candidate should be able to work independently and flexibly under general supervision.

Demonstrated maturity and sensitivity to the needs of individuals with mental health and substance use disorders, along with an understanding of HIPAA regulations regarding confidentiality, are essential.
