We are seeking a skilled and compassionate Registered Nurse (RN) to join our Elderplace team as a PACE/Elderplace Care Manager. In this role, you will be responsible for providing high-quality care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.
Key Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient
- Coordinate with healthcare providers, family members, and caregivers to ensure seamless care delivery
- Provide education and support to patients and their families on disease management, self-care, and wellness
- Collaborate with the interdisciplinary team to ensure comprehensive care and services
- Monitor patient outcomes and adjust care plans as needed
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association certification
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- 1 year of experience in a PACE organization, preferably in a care management or coordination role
- Bachelor's Degree in Nursing
- 1 year of acute care RN experience
