RN - PACE/Elderplace Care Specialist

We are seeking a skilled Registered Nurse to join our Elderplace team, providing high-quality care to our patients in their homes and communities.
Key Responsibilities- Deliver professional, independent nursing care to patients in accordance with the Oregon Nurse Practice Act.
- Assess, plan, order, give, delegate, teach, and supervise professional nursing care delivery to promote patients' optimal health and independence.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Provide education and support to patients and their families to promote health and wellness.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Relevant certifications applicable to the position, as specified by the Nurse Manager.
- Coursework/Training relevant to the position, as specified by the Nurse Manager.
- Bachelor's Degree in Nursing.
- 1 year of Home health RN experience.
