RN (Per Diem/On Call) - PACE/Elderplace

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Per Diem/On Call RN at PACE/Elderplace. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to our patients and their families.
Responsibilities- Provide direct patient care and support to patients with complex medical and psychosocial issues.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and evidence-based care.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Relevant certifications applicable to the position, as specified by the Nurse Manager.
- Bachelor's Degree in Nursing.
- 1 year Home health RN experience.
