Job Summary

We are seeking a skilled Community Care RN to join our PACE/Elderplace team. As a Community Care RN, you will provide and supervise professional, independent nursing care for Providence patients. Your care will be based in the nursing process and consistent with the Oregon Nurse Practice Act.

Key Responsibilities

  • Apply knowledge of assessing, planning, ordering, giving, delegating, teaching, and supervising professional nursing care delivery that promotes patients' optimal health and independence.
  • Provide and supervise professional, independent nursing care for Providence patients in the PACE/Elderplace program.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct regular assessments and evaluations to ensure the highest level of care and quality of life for patients.
  • Develop and maintain strong relationships with patients, families, and caregivers to ensure seamless care coordination and communication.

Requirements

  • Current Oregon Registered Nurse License
  • National Provider BLS - American Heart Association certification within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
  • Bachelor's Degree in Nursing (preferred)
  • 1 year of Home health RN experience (preferred)
