The Role:

We are seeking a dedicated Case Manager to join our team. As a Case Manager, you will be responsible for managing cases for your assigned clients to ensure they receive the necessary services for their overall well-being and in accordance with their individualized service plan (ISP).

Your Key Responsibilities:

  • Counsel and advise individuals or groups with Behavioral issues.
  • Develop and implement individual treatment plans.
  • Establish treatment goals and milestones with clients.
  • Prepare progress reports and letters for relevant parties.
  • Educate families and groups about treatments and prevention programs.
  • Facilitate referrals to other healthcare or counseling professionals.
  • Keep accurate and Medicaid documentation.
  • Complete group therapy
  • Other duties assigned.

About You:

We are looking for an experienced professional who has a strong background in case management, social work, or a related field. You should have experience working with an Electronic Health Record System and completing Medicaid billable progress notes. Additionally, you should possess excellent written and verbal communication skills, as well as a compassionate and caring demeanor.

Additional Requirements:

  • Valid driver's license with less than 4 points on it.
  • Insurance.
  • Minimum of High School Diploma or equivalent.
