Physical Therapist Home Health - Weekender

VNA Health Group is seeking a skilled Physical Therapist to join our dynamic team in Maple Heights, Ohio. As a Physical Therapist Home Health - Weekender, you will play a vital role in providing high-quality care to patients in their homes.

Responsibilities:
  • Conduct initial assessments and develop personalized treatment plans to improve patient mobility, endurance, and overall well-being.
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and caregivers, and recommendations for equipment or adaptations to the home environment.
  • Re-evaluate and revise treatment plans as needed, with physician approval.
  • Collaborate with other healthcare professionals to ensure comprehensive care for patients.
  • Submit timely and accurate documentation of patient care.
Qualifications:
  • Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred.
  • One year of experience in a supervised healthcare setting.
  • Licensed to practice physical therapy in the State of Ohio.
  • CPR certification required.
  • Valid Driver's License.
