Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Maple Heights, Ohio.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide home care therapy to individuals in need of restoration, mobility improvement, and pain relief.

Responsibilities:
  • Conduct initial assessments to determine patient treatment needs.
  • Develop personalized plans of care with measurable goals and specific discipline-related treatments.
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and/or caregivers, and recommendations for equipment or adaptation of home environments.
  • Re-evaluate and revise plans of care as needed with physician approval.
  • Facilitate total care services to patients by making appropriate referrals to other disciplines.
  • Submit all documents in a timely manner.
Qualifications:
  • Bachelor's Degree in Physical Therapy preferred.
  • One year of experience in a supervised healthcare setting.
  • Ability to work effectively with diverse cultural, social, and ethnic backgrounds.
  • Strong planning and organizational skills.
  • Ability to work well under pressure and maintain attention to detail.
  • Ability to maintain confidentiality.
Requirements:
  • Licensed Physical Therapist by the State of Ohio.
  • CPR certification required.
  • Valid Driver's License.
