Job Title: Home Health Physical Therapist

VNA Health Group is seeking a skilled Physical Therapist to join our team in Maple Heights, Ohio.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide home care therapy to individuals in need of rehabilitation and pain relief. As a Physical Therapist with VNA Health Group, you will be responsible for developing and implementing individualized plans of care, providing direct physical therapy services, and collaborating with other healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities:
  • Conduct initial assessments and develop plans of care for patients in need of physical therapy services.
  • Provide direct physical therapy services, including exercises, stretches, and other interventions to help patients achieve their goals.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care for patients.
  • Document patient progress and adjust plans of care as needed.
  • Participate in ongoing education and training to stay current with best practices in physical therapy.
Requirements:
  • Bachelor's degree in Physical Therapy or related field.
  • Current licensure as a Physical Therapist in the State of Ohio.
  • One year of experience in a supervised healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
