Key Responsibilities

  • Adhere to organizational policies, procedures, and relevant legal regulations.
  • Conduct thorough medication history assessments upon referrals from healthcare providers.
  • Develop and implement treatment plans based on patient diagnoses, which may include pharmacotherapy and lifestyle adjustments.
  • Identify and mitigate medication-related issues, including untreated conditions, inappropriate drug choices, and adverse reactions.
  • Initiate and modify drug therapies in accordance with established protocols, utilizing patient interviews and clinical data.
  • Oversee laboratory monitoring related to medication therapies, ensuring appropriate testing and interpretation of results.
  • Monitor patient outcomes to enhance therapeutic effectiveness and adherence while minimizing adverse drug reactions.
  • Educate patients about their health conditions, medication regimens, and compliance strategies.
  • Engage in continuous improvement of pharmaceutical care services.
  • Provide training and resources to healthcare providers and staff through various educational formats.
  • Conduct medication reviews and reconciliations to optimize pharmacotherapy.
  • Utilize pharmacy information systems and maintain accurate documentation in patient records.
  • Support a collaborative work environment that promotes partnership and teamwork.
  • Supervise pharmacy support staff, including technicians and interns.
  • Perform additional duties as necessary.

Qualifications

Experience:

  • At least 1200 hours of experience in collaborative drug therapy management within the last five years, or completion of an ASHP Accredited PGY-1 residency.
  • Preferred credentials include Board Certified Oncology Pharmacist (BCOP) or completion of a PGY-2 residency in oncology.

Education:

  • Pharmacy degree meeting the licensure requirements.

Licensure and Certification:

  • California Pharmacist License required upon hire.
  • Basic Life Support certification within three months of hire.
  • National Provider Identifier required upon hire.

Additional Requirements:

  • Successful completion of a competency assessment.
  • Extensive knowledge of pharmacological principles, medical terminology, and disease management.
  • Familiarity with pharmacy laws and regulations.
  • Experience in aseptic techniques and sterile product preparation.
  • Ability to communicate effectively and work collaboratively within a healthcare team.

Preferred Qualifications:

  • Doctorate of Pharmacy (Pharm.D.)
  • Completion of a PGY-2 clinical pharmacy specialty residency or fellowship.
  • Board certification in a relevant specialty area.
  • Advanced Practice Pharmacist License.
