We are seeking a skilled Licensed Practical Nurse (LPN) to join our healthcare team at Wellpath. As a key member of our patient care team, you will be responsible for delivering high-quality care to our patients, working closely with our registered nurses and other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Assessment and Planning: Assist in the assessment, planning, and implementation of individualized treatment programs for patients under the supervision of an RN.
- Clinical Care: Implement clinical and technical aspects of care in accordance with established policies, procedures, and protocols, while ensuring patient safety.
- Medication Administration: Administer medications and obtain diagnostic tests in accordance with the Health Care Practitioner's orders and protocols, using proper techniques and procedures.
- Procedure Assistance: Assist Health Care Practitioner in medical or minor surgical procedures and implement nutrition and therapeutic diet plans as ordered.
- Documentation: Document nursing encounters using the SOAP form of charting and communicates information to nursing staff and other relevant personnel.
- Education: Graduate from an accredited school of nursing.
- Experience: 1-2 years of acute medical experience.
- Licenses/Certifications: Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment. Must be able to obtain and maintain CPR certification.
