Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our healthcare team at Wellpath. As an LPN, you will play a vital role in delivering high-quality patient care and working collaboratively with our multidisciplinary team to achieve exceptional patient outcomes.
Key Responsibilities- Assessment and Planning: Assist in the assessment, planning, and implementation of individualized treatment programs for patients under the supervision of a Registered Nurse (RN).
- Clinical Care: Implement clinical and technical aspects of care in accordance with established policies, procedures, and protocols, ensuring patient safety and well-being.
- Medication Administration: Administer medications and obtain diagnostic tests in accordance with the Health Care Practitioner's orders and protocols, using proper techniques and procedures.
- Procedure Assistance: Assist Health Care Practitioners in medical or minor surgical procedures and implement nutrition and therapeutic diet plans as ordered.
- Documentation and Communication: Document nursing encounters using the SOAP form of charting and communicate information to nursing staff and other relevant personnel.
- Education: Graduate from an accredited school of nursing.
- Experience: 1-2 years of acute medical experience.
- Licenses/Certifications: Current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment and CPR certification.
