VNS Health is seeking a skilled Physical Therapist to join our team as a Float Physical Therapist. As a member of our interdisciplinary care team, you will practice independently in the community, conducting comprehensive assessments of patients and developing personalized rehabilitation therapy plans.
Key Responsibilities- Deliver high-quality, patient-centered care in the comfort of patients' homes or care facilities.
- Conduct thorough assessments to determine patient needs and develop effective treatment plans.
- Collaborate with healthcare professionals to ensure seamless care coordination.
- Continuously evaluate patient progress and adjust treatment plans as needed.
- Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
- Licensure to practice as a Physical Therapist in New York State.
- Valid driver's license required for all areas outside the borough of Manhattan.
- Competitive pay rates: $70-$75 per visit for areas outside the Bronx, $90-$95 per visit for the Bronx.
