Job Summary

The Certified Diabetes Care and Education Specialist (CDCES) plays a critical role in providing comprehensive diabetes care and support to our patients. This position requires a strong background in diabetes education and management, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Develops and implements individualized diabetes self-management education/training plans for patients
  • Provides educational interventions utilizing interactive training methods and maximizes collaboration with healthcare teams
  • Evaluates effectiveness of care management services by measuring attainment of learning objectives
  • Conducts follow-up assessments upon completion of DSME program services using outcome measures from the AADE7
  • Documents assessment data, educational plans, care services provided, and evaluation results in patient records

Requirements

To be successful in this role, you will need:
  • A minimum of three years of experience in diabetes care, education, and knowledge in program management
  • Certified Diabetes Care and Education Specialist (CDCES) certification in good standing with the Certification Board for Diabetes Care & Education (CBDCE)
  • Working experience with insulin pumps and continuous glucose monitoring technology is preferred
