Hospice Social Work Specialist Job Description

We are looking for a compassionate Hospice Social Work Specialist to join our team at Heartlinks. The ideal candidate will have experience in providing medical social work services to patients and families in a hospice setting.

Main Responsibilities:
  • Conduct thorough assessments of patient psychosocial status.
  • Collaborate with the interdisciplinary team to develop individualized care plans.
  • Communicate effectively with patients, families, and healthcare providers.
Required Qualifications:
  • MSW degree from an accredited institution.
  • Proven experience in hospice care or a related field.
  • Excellent communication and interpersonal skills.

Salary Range: $60,000 - $75,000 per year
