Our mission is to provide a range of social services to Los Angeles area communities, including Head Start programs, Upward Bound college prep programs, veterans services, homeless shelters, low-income housing programs, and drug and alcohol rehabilitation.

Job Summary and Purpose

The Senior Health Care Navigator will play a critical role in connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. This position requires a strong understanding of the VA health care system and community resources, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Conduct non-clinical assessments to identify Veterans' needs and develop Individual Needs and Services Plans (INSP) to navigate government benefit programs.
  • Work closely with Veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision making of their care.
  • Provide comprehensive case management and care coordination across episodes of care, including coordinating case conferences with residents, consulting with service teams, and monitoring progress of clients on INSP.
  • Act as an advocate for clients before judicial, community, social service, and administrative bodies as needed, and facilitate support groups and client participation as appropriate.
  • Collaborate with other disciplines involved in providing care, including medical, nursing, and administrative specialists, and case management personnel.
Requirements
  • Bachelor's Degree in Social Work, Public Health Administration, or related field, or four years of direct relevant experience.
  • Minimum three years of experience working with veterans in a community/veterans resource program.
  • Strong understanding of the VA health care system and community resources.
  • Excellent communication and interpersonal skills.
Preferred Qualifications
  • Masters Degree in related area.
  • LCSW preferred.
  • U.S. Veteran strongly preferred.
  • Existing relationships with VA or other community/veterans resource providers.
  • HMIS Database experience strongly preferred.
