About the Role

UnitedHealth Group is seeking a skilled In-Home Patient Care Specialist to join our team in providing high-quality healthcare services to patients in the comfort of their own homes. As a key member of our HouseCalls product, you will play a vital role in delivering comprehensive assessments and care management to patients across various settings.

Salary and Benefits

We offer an annual salary of $88,000 to $173,200 based on local labor markets, education, work experience, certifications, and other factors. In addition to your compensation, you'll enjoy a range of benefits, including a comprehensive benefits package, incentive programs, equity stock purchase opportunities, and 401(k) contributions. Job Responsibilities

As an In-Home Patient Care Specialist, you will be responsible for conducting thorough home assessments, evaluating patient medical history, reviewing systems, reconciling medications, taking vital signs, and performing comprehensive physical exams. Your expertise will also involve identifying diagnoses, formulating treatment plans, communicating findings with primary care physicians, and educating patients on disease processes, medication compliance, and health-related topics.

Requirements and Qualifications
  • An active NP license or the ability to obtain one by start date, with unencumbered licensure free from any disciplinary actions.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in the state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and sitting, standing, and kneeling to perform physical assessments.
Preferred Qualifications
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Maintenance of additional licensure in other geographic areas as requested.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Background in home healthcare or home visit experience.
