Responsibilities

The Regional Case Management Professional will perform the primary functions of assessment, planning, facilitation, and advocacy through collaboration with the member and other healthcare resources involved in the member's care. Key responsibilities include:

  • Coordinate all case management services within assigned areas to ensure appropriate coordination of services to CHPW members
  • Represent CHPW during regional meetings regarding care management/care coordination
  • Collaborate with HSD/CHPW leadership for presentation preparation

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience.
