Job Title: Social Service Assistant

Join Axesspointe Community Health Center as a Social Service Assistant and contribute to the well-being of our residents. As a key member of our team, you will provide essential support services to ensure a safe and stimulating environment.

Key Responsibilities:
  • Assist in the efficient running of the facility and its therapeutic programming.
  • Answer phones, direct calls, and take messages.
  • Keep the front office organized and maintain a list of office and facility supplies.
  • Conduct regular fire drills and check fire extinguishers.
  • Prepare rooms for incoming residents and maintain medication logs.
  • Process donations and complete check requests.
  • Prepare meals for residents and track therapeutic attendance.
  • Complete contact summaries and address potential client inquiries.
  • Accompany residents to appointments and promote anti-discriminatory practices.
  • Adapt to the environment and provide support to ensure residents experience an improved quality of life.
Requirements:
  • Ability to model open communication, fair treatment, and compassion.
  • Excellent communication skills for team members and clients.
  • Knowledge of community services and Microsoft Word skills.
  • Valid Ohio Driver's License and ability to meet agency requirements for driving insurability.
  • CPR/First Aid Certification preferred.
