Job Title: Adult Community Case Manager

We are seeking a dedicated and compassionate Adult Community Case Manager to join our team at Independence Association. As a case manager, you will play a vital role in supporting individuals with intellectual disabilities or autism to live fulfilling and inclusive lives.

Key Responsibilities:
  • Conduct thorough assessments of individuals' strengths and needs
  • Develop comprehensive plans to address service needs and promote independence
  • Provide advocacy, outreach, and resource coordination to support individuals and their support groups
  • Collaborate with individuals and their support groups to develop Person-Centered Plans and achieve goals
Requirements:
  • Bachelor's degree in a relevant field
  • At least 1 year of experience as an Adult Case Manager
  • Excellent communication and interpersonal skills
  • Ability to advocate for individuals with disabilities in various settings
  • Valid driver's license and ability to travel
