Job Title: Registered Nurse Case Manager

About the Role:

The Home Care Case Manager supports CHANS HomeCare's mission, vision, and values by exhibiting excellence, competence, collaboration, innovation, respect, patient and family-centered care, commitment to our community, and accountability.

Key Responsibilities:
  • Provide highly reliable, evidence-based, patient-family-centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of the effects of nursing care.
  • Travel to patients' homes to administer services and help patients maintain their independence.
  • Work with the provider, interdisciplinary team, the patient, and family to facilitate a plan of current care and coordinate any required follow-up care.
  • Collaborate with the patient and family to promote a balance of quality and cost-effective care.
Requirements:
  • Education: Bachelor's Degree preferred.
  • License/Certifications: Active nursing license, current, valid driver's license, and certification in Basic Life Support for Healthcare providers.
  • Experience: Negotiable.
  • Understanding of healthcare reimbursement preferred.
  • Proficient computer and communication skills.
