Job Summary:

Health First is seeking a highly skilled and experienced Nurse Manager to lead our Hospital at Home (HaH) program. As a key member of our team, you will be responsible for overseeing the day-to-day operations of the program, ensuring high-quality patient care, and promoting a culture of excellence within our organization.

Key Responsibilities:

  • Manage the HaH program, including patient identification, coordination with the Emergency Department team, and scheduling of associates and interdisciplinary teams.
  • Conduct disciplinary conferences and document individual counseling and disciplinary actions with associates.
  • Perform managerial responsibilities related to recruitment, hiring, coaching, development, and evaluation of staff.
  • Communicate policies and procedures to ensure compliance with Health First and governing expectations.
  • Facilitate high-quality, cost-effective outcomes, focused on whole-patient orientation and self-management decision support.
  • Provide appropriate delegation and supervision of patient care activities, collaborating with the hospital's Quality department to monitor patient outcomes.
  • Ensure compliance with The Joint Commission and other organizational and regulatory agency standards, with quality indicators and core measures.
  • Communicate timely with stakeholders, provide updates during daily team huddles, and promote interdisciplinary plan of care.
  • Assume 24-hour accountability for unit activities, ensuring schedules are completed on time and in compliance with approved staffing plans.

Requirements:

  • Bachelor's in Nursing
  • Two (2) years of acute care experience; One (1) year of managerial experience in a related field
  • Current Florida RN license
  • American Heart Association BLS Healthcare Provider certification
  • Eligible Level II background screening

Preferred Requirements:

  • Master's in Nursing
  • Two (2) years of managerial experience leading interdisciplinary teams, interfacing with hospitalists, and other hospital functions

Physical Demands:

  • Travel to/from various Health First locations and patient residences
  • Visual acuity and hand-eye coordination to perform clinical tasks
  • Able to lift, push, or move equipment weighing up to 25 pounds with or without assistance
  • Stand, walk, bend, and stoop for 90% of assigned shift
  • Respond appropriately to life-threatening and stressful situations
