Job Summary

Your Hiring Coaches is seeking a skilled Patient Care Manager to lead our clinical team and provide exceptional patient care. As a key member of our team, you will be responsible for managing patient episodes, coordinating care needs, and ensuring quality care is delivered by our clinical team.

Key Responsibilities
  • Communication and Coordination: Communicate and coordinate care needs reported by case managers, ensuring seamless transitions and continuity of care.
  • Schedule Management: Update schedules, ensure continuity of care, and guarantee quality care is delivered by the clinical care team.
  • Case Conference Facilitation: Facilitate case conferences, follow up on action items, and ensure effective communication among team members.
  • Clinician Development: Coach and mentor clinicians, including monthly 1:1 sessions to address development needs and hold them accountable to role-specific KPIs.
  • Voluntary Attrition: Manage voluntary attrition of assigned team members, acting as the main point of contact for support and working to develop solutions to potential personnel concerns.
  • Site Management: Supervise and direct the work of all team members, assign admit and evaluation visits, schedule visits, and ensure staff complete visits in a timely manner.
  • Clinical and Administrative Functions: Manage clinical and administrative functions, including recordkeeping and responding to phone referrals/inquiries in a timely, professional, and compassionate manner.
  • On-Call Nursing: Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Registered Nurse: Possess current licensure to practice nursing in the practicing state.
  • CPR Certification: Maintain CPR certification.
  • Home Care Experience: Previous experience in a home care setting with two years of management or supervisory experience, preferred.
  • Interdisciplinary Setting: Proven ability to work within an interdisciplinary setting.
  • Licensed Driver: Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
  • Professional Standards: Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
  • Nursing Judgment: Demonstrates excellent observation, nursing judgment, and communication skills.
