Job Summary

We are seeking a skilled Healthcare Manager - RN to join our team at UnitedHealth Group Inc. As a key member of our healthcare services team, you will play a critical role in supporting patients with high-risk health conditions to navigate the healthcare system.

Key Responsibilities
  • Identify and Support High-Risk Patients: Collaborate with healthcare providers and clinic staff to identify patients at high risk and develop personalized care plans to ensure optimal health outcomes.
  • Provide Patient Education and Empowerment: Act as an educator, resource, and advocate for patients and their families to empower them to take control of their health and well-being.
  • Coordinate Care Transitions: Support the transition of patients with chronic care conditions from inpatient to outpatient settings, ensuring seamless care coordination and minimizing healthcare disparities.
  • Collaborate with Multidisciplinary Teams: Work closely with healthcare providers, nurses, pharmacists, laboratory technologists, social workers, and other educators to ensure comprehensive care and optimal health outcomes.
  • Perform Clinical Tasks and Documentation: Perform accurate and timely documentation in electronic medical records, conduct medication reconciliation, and perform other clinical tasks within the scope of a RN license.
Requirements
  • Education and Experience: Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience, or equivalent combination of education and experience.
  • Licensure and Certification: Active and unrestricted RN license, current BLS certification, and 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Skills and Qualifications: Excellent verbal and written communication skills, solid interpersonal skills, ability to interact productively with individuals and multidisciplinary teams, and proficiency in electronic medical records.
Preferred Qualifications
  • Experience in Patient Education and Motivational Interviewing: Experience in patient education and motivational interviewing skills, self-management goal setting, and fluency in English and Spanish.
