Job Summary
We are seeking a compassionate and skilled Patient Care Specialist to join our team at our Rutland, VT location. As a Patient Care Specialist, you will work under the direction of Registered Nurses to provide direct and indirect patient and resident care.
Responsibilities
- Assist patients and residents with their daily living activities such as personal hygiene, bathing, dressing, etc.
- Report changes in patient and residents' condition, patient/family concerns or complaints to charge nurse and/or supervisor.
- Participate in the care planning process.

Requirements
