Healthcare Transition Specialist

At UnitedHealthcare, we're committed to simplifying the healthcare experience and creating healthier communities. As a Healthcare Transition Specialist, you'll play a vital role in ensuring a seamless transition for patients from institutional settings to community-based care.
Key Responsibilities- Assess and develop individualized care strategies to promote the most appropriate, least restrictive community-based setting.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Provide intensive psychosocial rehabilitation and transition services, including Cognitive Adaptation Training (CAT).
- Complete specified screening instruments on each member every six months in the program.
- Communicate and coordinate with member's assigned service coordinator twice monthly to ensure continuity of care.
- Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
- Bachelor's Degree in health, social services, or a related field, and 1+ years of relevant experience in assisting people in transitioning from institutional settings to the community.
- Active Social Worker license or Licensed Behavioral Health Professional in the state of Texas.
- 3+ years of experience working within the community health setting in a healthcare role.
- 1+ years of experience working with persons with a serious mental illness (SMI) and/or experience of mental illness.
- Master-level Social Worker (LMSW, LCSW) or Professional Counselor, Licensed Bachelor Social Worker (LBSW), Provisionally Licensed Psychologist (PLP), or Licensed Psychological Associate (LPA) licensed to practice in Texas.
- A background in managing populations with complex medical or behavioral needs.
- 5+ years of experience working with MS Word, Excel, and Outlook.
- Case management experience.
- 5+ years of experience documenting in an electronic documentation system.
- Certified Case Manager (CCM).
- Experience in discharge planning.
- Experience with arranging community resources.
- Bilingual in Spanish and/or Vietnamese.
