We are seeking a skilled Hospice Clinical Social Worker to join our team at AccentCare, Inc. As a medical professional, you will play a vital role in providing compassionate care to patients and their families.
Key Responsibilities- Provide clinical social work services to patients and their families, including assessment, counseling, and support.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Conduct home visits and provide emotional support to patients and their families.
- Document patient interactions and progress in a timely and accurate manner.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement.
- Master's Degree in Social Work or related field from an accredited institution.
- Valid license to practice as a clinical social worker in the state of operation.
- Medicare/Medicaid/Insurance specialty preferred.
- Ability to travel to all business locations.
- Competitive salary range: $80,000 - $93,000 per year.
