Family Bridges, Inc. is seeking a highly skilled Medical Social Worker to join our team. As a Medical Social Worker, you will be responsible for providing psychosocial assessment and case management services to our participants.
Key Responsibilities:- Screen potential program participants and conduct home visits to assess their needs.
- Develop and implement care plans to address the participants' psychosocial needs.
- Work collaboratively with the multidisciplinary team to develop treatment plans and provide individual and family supportive counseling.
- Refer participants and their families to community agencies or facilities as needed.
- Maintain accurate and up-to-date case management records.
- Masters in Social Work accredited by the Council of Social Work Education.
- 0-4+ years of social work experience, preferably in geriatric care.
- Experience working with frail seniors and in a multidisciplinary team setting is preferred.
- Bilingual in English and Cantonese/Mandarin is required, with the ability to communicate in one of the targeted Asian languages desired.
