POSITION OVERVIEW
The RN is skilled in planning, organizing, and overseeing home care services with a focus on community health education. The professional RN utilizes available resources to plan and direct services tailored to meet the needs of individuals and their families within their homes and communities.
  • Conducts an initial assessment of the patient and family to identify home care requirements, including a comprehensive physical assessment and history of current and past illnesses.
  • Regularly reassesses patient nursing needs.
  • Initiates the care plan and adjusts as necessary based on changes in patient status and needs.
  • Utilizes health assessment data to formulate nursing diagnoses.
  • Develops a care plan in collaboration with the patient and family to set goals based on nursing diagnoses, incorporating therapeutic, preventive, and rehabilitative nursing actions.
  • Implements appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the healthcare provider.
  • Delivers direct patient care as defined by the State Nurse Practice Act.
  • Provides counseling to the patient and family to address nursing and related needs.
  • Offers healthcare instructions to the patient as deemed appropriate based on assessment and care plan.
  • Identifies discharge planning needs as part of care plan development and implements prior to patient discharge.
  • Acts as Case Manager, taking responsibility for coordinating patient care for an assigned caseload.
  • Contributes to and participates in care coordination and patient-centered care as part of an interdisciplinary clinical care team, including nurses, therapists, CNAs, and MSWs.

QUALIFICATIONS
  • Associate's or Bachelor's degree with a preference for one year of home health care experience.
  • Valid AZ Nursing license in good standing.
  • Current CPR/BLS certification.
  • Fingerprint clearance card.
  • TB test results within the last year.
  • AZ Driver's license and proof of insurance with reliable transportation.
  • Excellent observational, verbal, and written communication skills, along with strong problem-solving abilities.
