As a Patient Care Navigator, you will work under the guidance of the Clinical Nurse Manager to provide tailored support to patients, families, and caregivers, assisting them in navigating the complexities of the healthcare system. Your role will be pivotal in ensuring patients have access to essential resources and services.
Key Responsibilities:
- Serve as a liaison between patients and healthcare providers, facilitating both clinical and supportive care services.
- Coordinate patient appointments with multidisciplinary care teams, ensuring seamless communication and collaboration.
- Act as a primary contact for patients, empowering them in managing their health conditions and treatment plans.
Qualifications:
Required:
- Graduation from an accredited nursing program.
- Valid Washington Registered Nurse License upon hire.
- National Provider BLS certification from the American Heart Association upon hire.
Preferred:
- Bachelor's Degree from an accredited institution.
- Certification in a related specialty area upon hire.
- Oncology Nursing Society Chemotherapy and Biotherapy Card.
- A minimum of 2 years of experience in Oncology.
