Job Summary

We are seeking a skilled Clinical Nurse Specialist to provide high-quality nursing care services, case management, treatment planning, teaching, counseling, and referral to clients and families in a home and/or community setting. The ideal candidate will have a current and valid license to practice as a Registered Nurse in the State of California and experience in a clinical and/or public health setting.

Responsibilities:
  • Provide a variety of nursing care services, case management, treatment plans, teaching, counseling, and referral to clients and families in a home and/or community setting.
  • Conduct comprehensive nursing assessments, develop individualized care plans, and evaluate patient outcomes.
  • Collaborate with other healthcare providers, professionals, and agencies to ensure seamless care coordination.
  • Develop and implement strategies to prevent communicable diseases and promote healthy behaviors.
  • Perform related duties as required.
Requirements:
  • Possession of a current and valid license to practice as a Registered Nurse in the State of California.
  • Experience in a clinical and/or public health setting.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Excellent communication and interpersonal skills, with the ability to teach and counsel patients, families, and staff effectively.
  • Ability to exercise independent judgment, interpret data, analyze situations, and draw logical conclusions.
  • Ability to respond effectively to emergency situations and prioritize tasks accordingly.
  • Ability to work effectively in diverse/multicultural environments and adapt to changing circumstances.
  • Strong organizational and time-management skills, with the ability to plan, organize, and prioritize multiple tasks and responsibilities.
