Job Summary

We are seeking a highly skilled Oncology Nurse Navigator to join our team at The University of Chicago Medicine. As a key member of our clinical team, you will play a vital role in providing exceptional patient care and ensuring seamless transitions throughout the disease process.

Key Responsibilities
  • Assess patients' clinical, emotional, spiritual, psychosocial, financial, and other needs
  • Facilitate resolution to patient needs and monitor follow-up
  • Educate patients and families on disease, treatments, side effects, and reportable signs/symptoms
  • Coordinate discharge planning with patients, families, social workers, and healthcare providers
  • Develop and coordinate patient education materials, clinical pathways, and quality studies
  • Participate in multidisciplinary consultations and tumor conferences
Requirements
  • Registered Nurse with a current Illinois license
  • Graduate of a National League for Nursing accredited School of Nursing with BSN
  • Minimum of two years hematology/oncology clinical nursing experience
