Key Responsibilities
  • Ensure timely discharge planning and medication/order management for designated patients.
  • Document patient information accurately and efficiently in electronic medical records (EMR).
  • Manage referral forms, applications, and logs within the Case Management department.
  • Handle Utilization Review and Management, Appeals, Denials, and Peer-to-Peer requests/set-up.
  • Facilitate high-level communication with providers and staff.
  • Conduct training for new hires on utilization management, documentation, and processes.
Requirements
  • BASIC LIFE SUPPORT (BLS) certification.
  • REGISTERED NURSE (RN) licensure.
  • ASSOCIATE'S DEGREE in a related field.
  • 3+ years of related experience.
