Key Responsibilities:

- Develop and implement individualized plans to promote client independence and well-being
- Collaborate with community resources and agencies to ensure comprehensive support services
Requirements: To be successful in this role, you will need excellent communication and interpersonal skills, with the ability to work effectively with clients, caregivers, and community resources. A strong understanding of care coordination principles and practices is essential, as is the ability to work in a fast-paced environment.
