Youth Behavioral Health Case Manager

The Case Manager is responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the client's behavioral health needs. This role coordinates care with referral sources, referred parties, and program treatment teams, ensuring seamless service delivery.
Key Responsibilities- Complete assessments and treatment plans for clients
- Follow CM timelines for monthly contact, quarterly home/community visits, and other required interactions
- Assist clients in finding services as clinically indicated and work to decrease behaviors to ensure stability in the community and home
- Complete referrals, prior authorizations, and other paperwork required by health plans to secure services for clients
- Coordinate and facilitate Child and Family Team (CFT) Meetings
- Collaborate with the clinical team to ensure accurate and current treatment plans, CFT Meetings, CALOCUS, State Demographics, and other required paperwork
- Collaborate and coordinate services with other involved agencies, such as Department of Child Safety, Probation and Parole Departments, Division of Developmental Disabilities, schools, and other providers
- Work effectively within the Arizona Recovery Model, adhering to the 12 principles, including Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, Respect for the child and/or family's unique cultural heritage, Services tailored to the family, Stability, and Timeliness
- Support the mission, vision, and values of Red Mountain Behavioral Health Services
- Bachelor's Degree (Preferred)
- Behavioral Health experience required
- Must be at least 21 years of age
- Must have a valid driver's license and be insurable in Arizona
- Must pass background criminal history and drug test
- Current fingerprint clearance card
- CPR/First Aid certification
- Clear TB test
- Clear MVD Report
- Emergency safety response trained
- Maintain all annual required trainings (Fraud and Abuse, Driver Refresher, and Self Administration of Medication, Falls Prevention, Infectious Disease)
Hybrid Position, Full Time, 40 Hours per Week, Hourly, Non-Exempt, Hourly Salary Range $20.00 - $23.00
