Role Responsibilities:

We are seeking a qualified individual to fill the role of Case Manager, specifically working with our SMI Adult demographic. This full-time position requires adaptability in scheduling to accommodate the needs of clients and their families.

The Case Manager will provide direct crisis intervention services, support, transportation, and educational resources to families and individuals, either independently or in collaboration with other in-home services. Responsibilities include conducting family needs assessments, making suitable referrals to partner organizations and community resources as required, ensuring continuity of care, and facilitating communication with various departments and collaborators.

Key Qualifications:

  • A bachelor's degree in a behavioral health-related field or compliance with BHT standards.
  • Experience with Severely Mentally Ill Adults and familiarity with the adult care system is essential.
  • Flexibility in work hours and the ability to meet clients in diverse settings such as their homes or community locations.
  • Possession of a Fingerprint clearance card or eligibility to obtain one.
  • A valid driver's license in Arizona with a clean driving record over the past three years.
  • Reliable transportation access is mandatory.
  • Bi-lingual capabilities (Spanish) are preferred.
  • Applicants must be at least 21 years of age at the time of employment.

  • Ability to maintain composure in crisis situations, work both independently and collaboratively, and demonstrate empathy towards individuals of all backgrounds.
  • Adherence to professional conduct in line with ethical standards and agency policies.
  • Fostering positive relationships with team members, stakeholders, and community partners.
  • For those driving for agency purposes, a valid Arizona driver's license and appropriate insurance coverage are required.
