Role Responsibilities:

We are seeking a qualified individual to fill the role of Case Manager, focusing on our SMI Adult demographic. This full-time position necessitates adaptability in scheduling to accommodate the needs of clients and their families.

The Case Manager will engage in direct service crisis intervention, provide support, facilitate transportation, and educate families and individuals, either independently or in collaboration with other in-home services. Responsibilities include conducting family needs assessments, making suitable referrals to partner organizations and community resources as required, ensuring seamless continuity of care, and aiding in communication with various departments within the agency or with external collaborators.

Key Qualifications:

  • A bachelor's degree in a behavioral health-related field or fulfillment of BHT requirements is essential.
  • Prior experience with Severely Mentally Ill Adults and familiarity with the adult care system is mandatory.
  • Flexibility in work hours and the ability to meet clients in diverse settings, including their homes and community locations, is required.
  • Possession of a Fingerprint clearance card or eligibility to obtain one is necessary.
  • A valid Arizona driver's license with no significant infractions in the last three years is required.
  • Regular access to dependable transportation is a must.
  • Bi-lingual candidates (Spanish) are encouraged to apply, with differential pay available.
  • Candidates must be at least 21 years of age at the time of employment.

  • Adherence to professional conduct in line with ethical standards and agency policies.
  • Fostering a professional demeanor and cultivating positive relationships with internal and external team members, stakeholders, and community partners.
  • For those driving on agency time, a valid Arizona driver license and appropriate liability insurance are required, along with a clean three-year state driving record.
