Job Summary

We are seeking a skilled Transplant Coordinator RN to join our team at Providence St Vincent Medical Center. As a key member of our transplant program, you will play a vital role in coordinating and managing the care of ventricular assist device (VAD) and cardiac transplant patients.

Key Responsibilities
  • Coordinate and manage the care of VAD and cardiac transplant patients, working closely with the program medical director and referring physicians to ensure optimal patient outcomes.
  • Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
  • Collaborate with other healthcare professionals to ensure seamless communication and coordination of care.
  • Provide education and support to patients and their families, empowering them to take an active role in their care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Education to meet certification, license or registration requirement.
  • Oregon Registered Nurse License upon hire.
  • VAD coordinator experience.
Preferred Qualifications
  • 3 years - Clinical experience with VAD patient care.
