MDS Coordinator

Avamere Communities is seeking a skilled MDS Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring the highest level of care for our residents.
Responsibilities- Plan, Schedule, and Revisit MDS: Complete the planning, scheduling, and revising of the Minimum Data Set (MDS), including the implementation of Care Area Assessments (CAAs) and Triggers.
- Review Patient Care Plans: Review patient care plans for appropriate goals, problems, approaches, and revisions based on nursing needs.
- Ensure Care Plan Awareness: Ensure all personnel involved in providing care to patients are aware of their care plans.
- Schedule Care Plan Conferences: Schedule and facilitate care plan conferences.
- Monitor PPS and OBRA Assessments: Monitor and guide the completion of PPS and OBRA assessments.
- Manage Utilization Review: Manage the Utilization Review (UR) process.
- Oversee MARs and Physicians Orders: Oversee MARs treatment, flow sheets, and physicians orders.
- Participate in Facility Surveys: Participate in facility surveys by authorized government agencies.
- Nursing Degree: Must possess a nursing degree from an accredited college or university.
- Current RN or LPN License: Must possess a current, unencumbered, active license to practice as a RN or LPN in this state.
- MDS Certification: MDS Certification preferred.
- RAI Process Knowledge: Knowledgeable and experienced in the RAI process, CAAs, RUG categories, and MDS 3.0.
- Nursing and Medical Practices: Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Electronic Medical Records: Experience with Electronic Medical Records and computer documentation systems.
- Confidentiality: Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
