The Opportunity

We are seeking a highly skilled Field Care Coordinator to join our High Risk team. As a Field Care Coordinator, you will play a critical role in supporting our patients with care outside of the traditional clinic setting. You will work closely with patients and their families to facilitate telehealth visits, conduct clinical assessments, and provide education on health and wellness.

Key Responsibilities
  • Facilitate telehealth visits in patients' homes, ensuring seamless communication with providers
  • Conduct standard patient assessments and documentation to facilitate complete visits
  • Provide education on health and wellness, including medication management and care planning
  • Coordinate care with patients, families, and outside care teams as needed
  • Connect patients to community resources and services to address social determinants of health
Requirements
  • At least 1 year of experience in home-based healthcare services or senior living care settings
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) certification preferred
  • Strong written and verbal communication skills, with ability to build trust and rapport with patients
  • Proven ability to work independently and as part of a team
  • Proficiency in computer technology and video platforms
