Position: Registered Dietitian

  1. ROLE SUMMARY

The Registered Dietitian is responsible for evaluating the nutritional requirements of clients and their caregivers. This role involves assessing dietary needs and formulating care plans that align with medical diagnoses and specific dietary restrictions. The Dietitian also supports clients requiring enteral nutrition and provides education to clients and caregivers on nutritional requirements, food sourcing, and menu development.

This position operates under the general guidance of the client's physician and collaborates closely with the Director of Food Services and other healthcare professionals. The Dietitian is entrusted with confidential information and is expected to uphold the privacy and security of Protected Health Information (PHI) and Electronic Protected Health Information (EPHI) in accordance with organizational policies and relevant regulations.

  1. PRIMARY RESPONSIBILITIES
  • Conduct initial and ongoing nutritional assessments for assigned clients based on established standards and evidence-based practices.
  • Develop and execute individualized nutrition care plans.
  • Communicate care plans to the Director of Food Services, RN case managers, and the client's physician, as well as other relevant personnel.
  • Obtain necessary physician orders for care.
  • Provide nutritional education and support to clients and their caregivers.
  • Facilitate patient education in enteral nutrition.
  • Monitor adherence to special diets and address nutritional non-compliance.
  • Offer nutritional counseling to clients and caregivers regarding optimal nutrition, food choices, and therapeutic diet adaptations.
  • Update the Director of Food Services and the interdisciplinary team on patient status and the effectiveness of care plans.
  • Document assessments and dietitian notes in the clinical records.
  • Engage with elderly and/or disabled clients regularly.
  • Foster a collaborative relationship with SEIU 1199.
  1. ADDITIONAL RESPONSIBILITIES
  • Demonstrate effective communication and listening skills in both team and individual settings.
  • Build and maintain cooperative working relationships.
  • Interpret a variety of written materials, data, and policies.
  1. QUALIFICATIONS
  • High school diploma is required.
  • Bachelor's degree in food and nutrition or dietetics from an accredited institution is required.
  • New York State license to practice as a Registered Dietitian is required.
  1. PHYSICAL & ENVIRONMENTAL REQUIREMENTS
  • Work with clients in their residences.
  • Able to lift up to 25 pounds.
