  1. Key Responsibilities:
  • Evaluate, diagnose, and provide treatment for speech, language, communication, and swallowing disorders.
  • Maintain clear, accurate, and timely documentation for all patient interactions and treatments.
  • Adhere to ASHA standards for ethical and professional practice.
  • Formulate individualized treatment plans in compliance with facility protocols, ensuring alignment with recognized professional standards. Complete all necessary documentation punctually, including evaluations, recertifications, progress notes, and care plans.
  • Promptly inform the medical team about initial treatment plans and any changes in patient conditions. Ensure all physician directives are current and properly implemented.
  • Deliver high-quality therapy as outlined in the treatment plan, following accepted professional practices.
  • Effectively communicate any concerns regarding patient progress to the patient, their family, and the interdisciplinary team. Revise treatment and care plans as necessary.
  • Collaborate with the Director of Rehabilitation Services on patient and staff matters, equipment needs, and departmental objectives.
  • Participate in professional development opportunities to stay current with industry standards and practices. Ensure all licensure and continuing education requirements are met in a timely manner.
  • Supervise Graduate Therapists (with NYS Permit), Clinical Fellowship Year candidates, and student interns as applicable.
  • Comply with attendance and dress code policies consistently.
  • Perform additional related duties as required within the department and organization.
  • Practice safe working habits and adhere to all safety protocols.
  • Ensure the safety, security, and well-being of our residents.
  1. Position-Specific Competencies:
    • Comply with OSHA and DOH health and safety regulations.
    • Utilize universal precautions in all interactions.
    • Exhibit safe resident care practices.
    • Employ proper body mechanics for self and residents.
    • Demonstrate understanding of indications and contraindications in resident care.
    • Administer treatments within the physiological limits of the resident.
    • Seek appropriate assistance when necessary.
    • Provide timely support to staff and residents.
    • Maintain a safe working environment.
    • Possess basic computer skills, including word processing and internet navigation.
  2. Working Conditions:
    • Must demonstrate empathy and sensitivity when working with the elderly population.
    • Potential exposure to human bodily fluids.

  1. Personal or Physical Requirements:
  • The physical demands described are representative of those that must be met by an Associate to successfully perform the essential functions of this role. Reasonable accommodations may be made for individuals with disabilities.
  • Ability to speak, read, and write in English, as well as communicate in the predominant language of the residents.
  • Position requires bending, stooping, pushing, and pulling.
  • Must be able to stand or walk for extended periods.
  • Ability to lift up to 30 lbs. with assistance as needed.
  • High likelihood of exposure to human bodily fluids.
  • Working with the elderly necessitates respecting the privacy, dignity, and confidentiality of all residents.
  1. Education:
  • Master's degree from an accredited speech-language pathology program.
  • Licensed Speech-Language Pathologist in New York State.
  • Certificate of Clinical Competence in Speech-Language Pathology from the American Speech-Language Hearing Association (ASHA) is preferred.
  • Strong interpersonal and communication skills.
  • Maintain confidentiality in accordance with facility policies.
