Registered Dietitian

  1. JOB SUMMARY

The Registered Dietician conducts assessments of Weinberg Campus clients and caregivers. Assesses nutritional needs and implements care plans consistent with diagnoses and dietary needs/restrictions. Promotes nutritional support for enteral nutrition clients. Educates clients and caregivers regarding nutritional needs, obtaining adequate food sources, developing menus, assisting with setting up home delivered needs.

Performed under the general supervision of the client’s physician. Employee interacts with and takes direction from the Director of Food Services and interacts with other health care professionals. Has access to confidential information as necessary to perform job duties. Employee is expected to respect the right to privacy and security of Protected Health Information (PHI) and Electronic Protected Health Information (EPHI) in accordance with Weinberg Campus Policy and all applicable laws and regulations.

  1. PIMARY JOB DUTIES
  • Conducts initial and periodic nutrition assessments on assigned clients according to developed standards and evidence-based practice.
  • Develop and implement client specific nutrition care plans.
  • Communicates plan for care to Director of Food Services, RN case managers, client’s physician, and other individuals with a “need to know” as described by policy, laws, and regulations.
  • Obtain physician orders for care.
  • Provides nutrition education and guidance to clients and caregivers.
  • Conducts patient education/teaching in areas of enteral nutrition.
  • Monitors special diets and nutritional non-compliance.
  • Nutritional counseling for clients and caregivers regarding proper nutrition, food selection, adaptation of therapeutic diet plans, and adequate food sources.
  • Provides updates regarding patient status, effectiveness of plan of care to Director of Food Services, RN case managers, and an interdisciplinary team.
  • Document assessments and dietitian notes in the clinical record.
  • Frequently work with the frail elderly and/or handicapped clients.
  • Develop and maintain collaborative relationship with SEIU 1199.

  • Read and understand/interpret a variety of correspondence, written materials, data, and policies and procedures.

  • Proficiency in use of Electronic Medical Records (EMR)
  • Ability to effectively manage multiple, competing, and high priority projects with varying deadlines
  • Ability to exercise independent judgement
  • Absolute confidentiality skills

  • Strong organizational and interpersonal skills
  • Proficiency in developing reports to meet the needs of the RCCHHA department as required
  • Ability to prioritize workload
  • Ability to function well in fast paced environment

  1. JOB SPECIFICATIONS:
  • High school diploma required
  • Bachelor’s degree from an accredited college with major studies in food and nutrition or dietetics required
  • NYS license to practice as a Registered Dietitian

  1. PHYSICAL & ENVIROMENTAL REQUIREMENTS:
  • Work with client’s in their homes
  • Able to lift 25 pounds
