Medical Case Manager

About the Role
We are seeking a highly skilled Medical Case Manager to join our team at Interim HealthCare of the Twin Cities. As a key member of our interdisciplinary team, you will be responsible for providing comprehensive case management services to patients in their homes.
Key Responsibilities
- Perform thorough assessments and develop individualized care plans to meet each patient's unique needs.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
- Monitor patient progress and adjust care plans as needed to optimize health outcomes.
Requirements
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in the State of MN.
- A minimum of 2 years of experience in case management or a related field, preferably with a focus on home health care.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- CPR certification is required.
Benefits
- A competitive salary range of $84,905 - $101,233 annually, commensurate with experience.
- A generous hiring bonus of $6,000 upon completion of a successful probationary period.
