Job Overview

We are seeking a dedicated and experienced Medical Assistant to join our Chronic Care Management team at Woodlands Primary Healthcare, providing support to doctors and their patients. The successful candidate will play a vital role in delivering quality care to patients with chronic conditions.


Salary and Benefits

The hourly salary for this position ranges from $18.00 to $21.00, commensurate with experience and certifications. We also offer health insurance, 401K, and PTO benefits.


Job Responsibilities
  • Collaborate with physicians to create care plans for patients with chronic conditions
  • Assist in patient education and self-management support
  • Coordinate follow-up appointments and ensure patients adhere to treatment plans
  • Monitor patient progress and communicate any concerns to the healthcare team
  • Document patient encounters and maintain accurate records in the electronic health record system
  • Conduct medication reconciliation and assist with prescription refills
  • Perform basic patient assessments and vital sign measurements

Requirements
  • Completion of a medical assistant program (not required if you have experience in the medical field)
  • Proficiency in medical terminology and EMR systems, specifically e-clinical works
  • Excellent communication and interpersonal skills
  • Team player and able to work with others to achieve department goals
  • Ability to multitask in a fast-paced clinical environment
  • Compassionate and empathetic approach to patient care
  • Bilingual due to the high number of Spanish-speaking patients

Working Conditions
  • Hours of work: Monday - Friday, 8-5
