CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support in the comfort of their own homes.
Key Responsibilities- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, prevention, and health promotion.
- Conduct thorough assessments, identify patient needs, and develop effective care plans in collaboration with the interdisciplinary team.
- Administer medications, perform treatments, and provide education to patients and their families on disease management, self-care, and health maintenance.
- Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination and transitions.
- Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
- Graduate of an accredited Licensed Practical Nursing program or vocational nursing school.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
