About the Role

We are seeking a highly skilled and organized Patient Services Coordinator to join our team at CenterWell Home Health. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of high-quality patient care.

Key Responsibilities
  • Scheduling and Coordination
    • Manage patient schedules, ensuring timely and efficient coordination of care.
    • Edit schedules as needed, reassigning patients to ensure continuity of care.
    • Update agent unavailability in the worker console.
  • Infection Control and Reporting
    • Initiate infection control forms as needed.
    • Send completed reports to the HR department for upload in the worker console.
  • Communication and Support
    • Serve as a backup during busy periods, receiving calls from field staff and assisting with weekly case conferences.
    • Refer clinical questions to the Branch Director as necessary.
  • Log Maintenance and Reporting
    • Maintain the client hospitalization log, including entering coordination notes.
    • Send electronic logs to all relevant staff.
  • Schedule Management
    • Complete requested schedules as tasks appear on the action screen.
    • Ensure staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
  • Reporting and Scheduling
    • Run scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report.
    • Prepare weekly agent schedules, reviewing for productivity and geographic issues.
  • Administrative Support
    • Assist with internal patient transfers between branch offices.
    • Verify visit paper notes in the scheduling console as needed.
  • Clinical Support (Optional)
    • Receive lab reports, assess for normality, and fax copies to doctors and medical records.
    • Initiate Employee/Patient Infection Reports as necessary.
    • Perform patient visits and participate in on-call rotations (if clinically qualified).
Requirements
  • Experience and Skills
    • Excellent communication and interpersonal skills.
    • Strong organizational and time management skills.
    • Ability to manage and prioritize multiple assignments.
  • Education and Certifications
    • At least 1 year of home health experience.
    • Prior packet review/QI experience preferred.
    • Coding certification is preferred.
  • Professional Requirements
    • Valid state driver's license and automobile liability insurance.
    • Current licensure in the state of employment (if applicable).
