As a skilled Home Health Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive, skilled nursing visits to home health patients, addressing their unique needs and concerns.
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans, ensuring seamless coordination of care.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to optimize patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and seamless care transitions.
  • Monitor patient progress, adjust care plans as needed, and provide education and support to promote self-management and independence.
  • Supervise and mentor home health aides and other care team members to ensure high-quality, patient-centered care.
  • Document patient care and outcomes accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related setting
  • Strong med-surg, ICU, ER, or acute care experience
  • Home health experience a plus
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational, communication, and interpersonal skills
  • Valid driver's license, auto insurance, and reliable transportation
